What is Care-Connect at Home?

Care-Connect at Home is a licensed home health agency providing PAS (Personal Assistance Services) to geriatric clients in their own home environments, which can be their house, retirement community, apartment, assisted living apartment, nursing facility, or any other place that a person may call “home.”

Care-Connect at Home is an affiliated program of Lutheran Social Services of the South, Inc.

What services do your caregivers provide?

Their main job is to create safety and companionship by providing meals, housekeeping, medication reminders, and transportation, among other services.

Services include:

  • Bathing, dressing, and grooming assistance
  • Light meal preparation
  • Light housekeeping, laundry and linen changes
  • Transportation to appointments
  • Medication reminders
  • Incontinence care
  • Companionship
How are home visits scheduled?

With visit days and times chosen by the client or a loved one, the schedule can be flexible allowing for medical appointments and social events. Our services are available 24 hours a day, 7 days a week. There is a three (3) hour minimum for the day.

What makes Care-Connect home care unique?

Care-Connect at Home is the only nonprofit organization in the area providing this type of specific care.
Because of our small size we are able to “hand-pick” our care attendants for each client with the goal of matching personalities and needs. However, our care attendants are cross-trained with each client so if someone is out for any reason, the client is not receiving a stranger into their home. When service starts, the care attendant is escorted to that first visit by the Program Manager. This allows the Program Manager and care attendant to review the Service Plan created specifically for the needs of that client.

Who are the caregivers?

Our care attendants are meticulously screened with references, criminal background, drug screen, and driving record thoroughly examined prior to hire. We hire from personal recommendations.
We have two levels of employee interviews, and an extensive orientation process.

How do you charge for services?

Because we are a non-profit, faith-based program, we offer our services at very competitive rates. We have a 3-hour minimum per day for services and the convenience of monthly billing, which may be paid by check or credit card. We can also assist with your long-term care insurance billing. If applicable, you may be covered by the Veterans Affairs Aid and Attendance Pension program.
